✅ How to Fill Annexure ‘J’ Form
1️⃣ Applicant Details:
Step 1 —Enter the full name of the applicant (husband or wife) in the first box.
Step 2 —Fill in the full residential address in the “Full Address” box.
Step 3 —Write your spouse’s name in the relevant boxes wherever required.
2️⃣ Declaration Details:
Step 1 — Confirm that you are married and living together as a couple.
Step 2 — Mention that your passport will be issued/reissued with your spouse’s name.
3️⃣ Joint Photograph:
Step 1 — Attach a latest joint passport-size photograph of husband and wife in the photo box provided.
Step 2 — Both must self-attest on the photograph (sign partly on the photo and partly on the form).
4️⃣ Place and Date:
Step 1 — Write your place of signing (city name) in the “Place” field.
Step 2 — Fill in the current date in DD/MM/YYYY format in the “Date” field.
5️⃣ Signatures:
Step 1 — Both husband and wife must sign at the respective signature places below the declaration.
Step 2— Fill in Name, Aadhaar Number, Voter ID Number, Passport Number for both husband and wife in the respective boxes.
6️⃣ Printing the Form:
Step 1 —After filling all details, click “Print / Save as PDF”.
Step 2 —The filled details will be shown in blue ink and fixed text will be in black.
Step 3 —Save or take a printout of the final PDF.
7️⃣ Submission at Passport Office:
Step 1 —Submit the printed Annexure ‘J’ along with your passport application at the Passport Seva Kendra (PSK) or Passport Office.
Step 2 —This form is used as a joint declaration in lieu of a marriage certificate.
